Job summary

Eben Ezer Lutheran Care Center in Yuma, CO is seeking a Part-Time QMAP caregiver for direct neighbor care in a non-remote setting. Responsibilities include assisting with bathing, meal service, grooming, medications, and transportation coordination per care plans.

The role requires CPR/First Aid, QMAP certification, and basic computer skills; high school diploma or GED preferred. Flexible shifts include 8-12 hour blocks with PRN opportunities.

Qualifications

• High school graduation or GED preferred.
• CPR and First Aid certification, and QMAP medication certification, and computer experience preferred.

Responsibilities

• Assist with bathing, laundry, housekeeping, meal service, grooming, reminders, and toileting as outlined in care plan.
• Document neighbor information per regulations and HIPAA standards.
• Coordinate medical and non-medical appointments and transportation for neighbors.
• Provide mental, emotional, and physical support to neighbors.
• Maintain privacy and dignity of neighbors.
• Communicate neighbor condition with physicians, families, case manager, and Administrator as necessary.
• Operate glucometer and blood pressure equipment for basic assessments.
• Ensure data entry accuracy for orders and records.
• Limit and manage time effectively; work independently or in a team.
