Job summary

Table Rock Post Acute in Medford, OR is seeking a dedicated Physical Therapist Assistant to work under a licensed PT. You will help residents regain movement and manage pain through prescribed exercises and manual techniques.

This on-site, full-time role requires helping with ambulation, documenting care, and following infection-control policies. You will collaborate with staff to implement individualized treatment plans and promote overall wellness for residents.

Qualifications

• Graduate of an accredited PTA program.
• Current state PTA licensure.
• Valid CPR certification.
• Six months experience in a long-term care environment preferred.

Responsibilities

• Assist residents with ambulation and mobility.
• Contribute to PT effectiveness by identifying care issues and commenting on options.
• Assess patient health via interviews, exams, and review of histories.
• Review PT interpretations of evaluations and test results.
• Document patient care in charts and records.
• Administer manual exercises and guide activities and use assistive devices.
• Instruct and counsel patients on regimens and wellness.
• Provide continuity of care per the patient management plan.
• Maintain a safe, clean workspace and follow infection-control policies.
• Monitor treatment trends and alignment with production objectives.
• Ensure punctuality and reliability for shifts.
