Job summary

Cascadia Healthcare is seeking a Physical Therapist for Table Rock Post Acute in Southern Oregon. The role focuses on restoring function, reducing pain, and promoting mobility for residents in a skilled nursing setting. You will develop individualized treatment plans and collaborate with a multidisciplinary team.

Full-time with PRN options, offering hourly pay in the mid-$50s. Requires PT licensure in the state and CPR certification. Prior long-term care experience is preferred.

Qualifications

• Bachelor’s, Master’s or Doctorate Degree from a college or university with an accredited Physical Therapy program.
• Current Physical Therapist license in the state employed.
• Current CPR certification.
• Six months experience in a long-term care environment preferred.

Responsibilities

• Establishes and administers a treatment plan with goals per patient capacity under physician direction.
• Assesses patient needs; plans for, evaluates and modifies care to meet PT goals.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
• Sets realistic and achievable goals for patients/residents.
• Documents and records patient progress per regulations.
• Educates patients and families in appropriate therapy methods.
• Optimizes function by screening residents and coordinating care.
• Communicates patient status to family and other professionals.
