San Jacinto Valley Post Acute in Hemet, California is seeking a compassionate Physical Therapist to join our dedicated team. You will evaluate residents, develop individualized treatment plans, and help patients achieve mobility and independence through evidence-based rehabilitation.

We offer a full-time position with flexible scheduling, continuing education, and a supportive interdisciplinary team to deliver patient-centered care and measurable outcomes for residents’ quality of life.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current California Physical Therapist license, or eligibility for California licensure.
• Current CPR certification preferred.

Responsabilités

• Evaluate residents and develop individualized treatment plans based on clinical findings and goals.
• Provide therapeutic interventions to improve strength, mobility, balance, endurance, and functional independence.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with physicians, nursing staff, therapy team members, residents, and families to ensure coordinated care.
• Educate patients and caregivers on treatment plans, exercises, and strategies to support recovery and long-term wellness.
• Maintain accurate and timely documentation in accordance with regulatory and facility requirements.
• Ensure compliance with all applicable professional, ethical, and safety standards.
