Job summary

San Jacinto Valley Post Acute in Hemet, CA is seeking a compassionate Physical Therapist to evaluate, develop, and implement individualized treatment plans that improve mobility and independence for residents in a post-acute setting.

You will work within an interdisciplinary team to deliver patient-centered care, educate patients and families, and ensure documentation and compliance with facility standards.

Qualifications

• Graduate from an accredited physical therapy program.
• Current California Physical Therapist license or eligibility.
• CPR certification preferred.
• Experience in skilled nursing, rehabilitation, outpatient, or post-acute care is preferred.
• New graduates are encouraged to apply.

Responsibilities

• Evaluate residents and develop individualized treatment plans based on clinical findings and goals.
• Provide therapeutic interventions to improve strength, mobility, balance, endurance, and functional independence.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with physicians, nursing staff, therapy team members, residents, and families for coordinated care.
• Educate patients and caregivers on treatment plans, exercises, and strategies to support recovery and long-term wellness.

Position Summary

The Physical Therapist is responsible for evaluating, developing, and implementing individualized treatment plans to improve mobility, reduce pain, restore function, and enhance quality of life. This role collaborates closely with an interdisciplinary team to deliver outstanding patient-centered care.

Essential Responsibilities

• Evaluate residents and develop individualized treatment plans based on clinical findings and goals.
