Job summary

PACS in Hemet, CA is seeking a dedicated Physical Therapist to evaluate residents, develop individualized treatment plans, and collaborate with an interdisciplinary team to enhance mobility and independence.

You will deliver therapeutic interventions, monitor progress, educate patients and families, maintain compliant documentation, and uphold professional standards in a full-time post-acute care setting.

Qualifications

• Graduate of an accredited Physical Therapy program and licensed or eligible for California licensure.
• Current CPR certification preferred.
• Experience in skilled nursing, rehabilitation, outpatient, or post-acute care is preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide interventions to improve strength, mobility, balance, and independence.
• Monitor progress and modify plans as needed.
• Collaborate with physicians, nursing, therapy staff, residents, and families.
• Educate patients and caregivers on treatment plans and exercises.
• Maintain accurate documentation per regulatory and facility requirements.
• Adhere to professional, ethical, and safety standards.
