Crossroads At Delta Memory is seeking a compassionate Personal Care Provider to assist residents with daily living activities in a respectful, dignified setting.

You will support residents with personal care, mobility, meal service and timely documentation, using the iPad, while collaborating with a care team to maintain safety and comfort in Delta, Colorado.

A flexible schedule, reliability and strong communication with residents and staff are essential.

Qualifications

• Assist residents with daily living activities in a respectful, dignified manner.
• Maintain cleanliness of all areas within the Crossroads facility and care environment.
• Ability to document codes and notes using an iPad and electronic systems.

Responsibilities

• Perform resident care with dignity and individuality.
• Maintain cleanliness of care areas and equipment.
• Respond to emergency and routine calls via pendant system.
• Assist in dining room setup, serving, and cleanup.
• Make beds and check rooms for trash and soiled laundry.
• Notify supervisors of issues affecting safety, service or quality.
• Demonstrate understanding of care terminology and resident needs.
• Adhere to attendance policies and schedules.
