Job summary

Anchor Health, based in California, seeks a Pediatric Hospice RN Case Manager to deliver professional nursing care for pediatric patients. You will develop and assess care plans with the IDT, oversee plan implementation, and ensure timely communication with families and the primary MD.

Travel to patient visits is required, and on-call duties are part of the role. The role emphasizes coordination, documentation, and adherence to hospice standards.

Qualifications

• Must have an Associate Degree in Nursing and an active CA RN license.
• Bachelor degree preferred for advancement.
• CPR/BLS certification required/commonly maintained.
• CA driver's license with proof of insurance is preferred/believed.
• Bilingual abilities are beneficial.

Responsibilities

• Develops written plan of care in collaboration with the IDT (interdisciplinary team).
• Oversees implementation and evaluation of the plan to meet patient/family needs.
• Supervises care provided by LVN and HA per policy and regulatory requirements.
• Communicates changes to IDT and primary MD; maintains timely documentation in electronic charting.
• Provides caregiver education and orientation to assigned personnel; participates in 24/7 on-call coverage.
