Job summary

Conviva Senior Primary Care is seeking a Patient Transport Driver to plan and provide safe, efficient transportation for patients to and from medical appointments. You will work within Humana's care network to support accessible, timely care.

You will contact patients by phone to arrange pick-up times, assist with wheelchair mobility, and report any schedule changes to Supervisors. A valid driver’s license and clean driving record are required.

Qualifications

• Experience driving a commercial vehicle.
• Basic computer and smartphone/tablet skills.
• Ability to transport patients safely, including wheelchair assistance.
• Flexible schedule with potential overtime.
• Good driving record and knowledge of local streets.
• Must maintain patient confidentiality.
• Must have valid state driver's license and personal vehicle insurance (100/300/100).
• TB screening is required; Florida associates need ACHA Level II clearance.

Responsibilities

• Contact patients by phone and arrange pick-up times.
• Transport patients to appointments.
• Coordinate with Transportation Supervisors by radio for schedule changes.
• Assist patients entering/exiting vehicle; operate lift for wheelchair users.
• Maintain daily logs of mileage, trips, passengers, and destinations.
• Check and maintain van daily; ensure safety and cleanliness.
