Job summary

Conviva Senior Primary Care in Macon, GA is seeking a Patient Transport Driver to plan and provide safe transportation for patients to medical appointments. You will contact patients by phone to arrange pick-up times and transport them to physicians' offices, staying in touch with supervisors by radio for schedule changes.

Responsibilities include assisting patients entering/exiting the vehicle, operating lift gates for wheelchairs, and keeping daily logs of mileage and trips.

Qualifications

• Professional experience with commercial vehicle as a Driver.
• Basic Computer and Smartphone/Tablet skills.
• Ability to transport patients safely and efficiently, including wheelchairs.
• Ability to work a flexible schedule with potential overtime on weekends/evenings.
• Good driving record and knowledge of local streets/addresses.
• Maintain patient confidentiality at all times.
• Must have basic mechanical skills to inspect vehicles daily.
• Florida associates: ACHA Level II Background clearance.
• TB screening as part of Humana/Senior Bridge program.

Responsibilities

• Contact patients by phone and arrange pick-up times.
• Transport patients to doctor's appointments.
• Stay in contact with Transportation Supervisors by radio for schedule changes.
• Assist patients to enter/exit vehicle and operate lift gate for wheelchairs.
• Maintain daily mileage, trips, passengers, and destinations logs.
• Check and maintain van daily for mechanical safety and cleanliness.
