Responsibilities

• Obtains and reports basic patient physiological data (vital signs, weight, etc.).
• Monitors fluids intake and output and documents.
• Performs duties under supervision of licensed staff and assists with ADL.
• Keeps patient charts organized and ensures timely documentation.
• Responds to patient calls and coordinates with staff to meet patient needs.
• Communicates effectively with patients, families, and healthcare team.
• Maintains safety and infection control practices as per policy.
