Qualifications

• BLS Basic Life Support preferred; must obtain before direct patient care
• Nursing aide training or verified nursing assistant program required (Ohio LTC option) or CNA/Nurses Aide certification
• Recent experience in acute care, physician's office, home health, hospice, or long-term care preferred

Responsibilities

• Provide basic patient care: comfort, vital signs, hygiene, mobility support
• Act as liaison between patient and nurse to report changes
• Deliver high-quality customer service to patients and families
• Document patient care accurately and timely
• Participate in hand-off communications at shift changes
• Perform delegated tasks once competency is validated
• Assist with patient transfers using safe techniques
• Promote independence and individualized care based on patient preferences
