Job summary

Responsibilities

• Provides clear, timely communication to the RN regarding observations and changes seen in patients.
• Provides constant observation of high-risk patients as assigned by the Resource Nurse or designee.
• Participates in the development and maintenance of a safe environment; uses interventions to de-escalate patients and participates in restraints as directed.
• Assists patient with daily living activities such as personal hygiene, ambulating, meals and nourishment.
• Assists in moving, lifting, positioning, and transporting patients using proper safety techniques.
• Maintains patient safety by performing safety checks and following restraint policies when caring for restrained patients.
• Takes and records vital signs and measures patient weight/height.
• Performs EKGs, POC tests and bladder scanning; collects and prepares specimens for lab transport.
• Performs other unit procedures and checks equipment, cleans equipment, stores supplies.
• Assists RN/LIP with treatments by preparing patient, supplies, positioning, and emotional support.
• Observes, measures and records intake; refills tube feeding bags; manages outputs from various sources.
• Demonstrates sensitivity and responsiveness to patients' needs and builds rapport and positive relationships.
• Provides respectful, culturally and age-appropriate communication with patients, staff, and visitors.
• Replenishes patient and unit supplies and equipment as needed.
• Requests assistance from identified resources when more direction is needed.
