Job summary

Heritage Trails Nursing and Rehabilitation Center is seeking a dedicated Certified Nursing Assistant to provide routine daily nursing care to residents per their care plans. You will identify residents, document care, report changes, and assist with transfers while maintaining a respectful, safe, and confidential environment.

The role requires following facility policies, accurate record-keeping, and teamwork with nursing staff to ensure comfort and well-being of residents.

Qualifications

• Ability to read, analyze, and interpret general business materials.
• Ability to write reports and procedures.
• Ability to respond to questions from managers and employees.

Responsibilities

• Provide routine daily nursing care per resident care plan.
• Identify residents using wristband/photo ID before procedures.
• Report changes in condition to Nurse Supervisor/Charge Nurse promptly.
• Record entries accurately on flow sheets and charts.
• Assist with transferring/moving residents and daily activities.
• Keep resident rooms clean, stocked, and ready for use.
• Maintain resident confidentiality and report any incidents.
