Heritage Trails is seeking a compassionate Certified Nursing Assistant to provide routine nursing care following assessment plans and supervising directions. You will identify residents, document care, and assist with daily activities while maintaining a warm, safe environment.

Qualifications

• Must possess a minimum of a 10th grade education.
• CPR certification preferred.
• Ability to read, analyze, and interpret general business information.
• Ability to perform basic math for patient care tasks.

Responsibilities

• Identify residents before administering treatments using wristband or photo ID.
• Report changes in residents' condition to Nurse Supervisor/Charge Nurse.
• Record entries on flow sheets, notes and charts.
• Assist residents with daily functions and personal care as directed.
• Maintain confidentiality of resident information and report any unauthorized disclosures.
