Job summary

HERITAGE TRAILS is seeking a Certified Nursing Assistant to provide routine daily nursing care to residents according to care plans. You will identify residents, assist with daily living activities, and support smooth unit operation.

The role requires adherence to policies, confidentiality, and timely reporting to supervisors. A CNA license (state-required) and CPR may be preferred; minimum 10th grade education is required. Joint teamwork with the nursing staff is essential.

Qualifications

• Language skills, including reading and writing.
• Basic math and measurement skills for health-related tasks.
• Ability to reason and follow written and verbal instructions.

Responsibilities

• Identify residents with wristbands or photo cards before care and treatments.
• Record entries clearly on flow sheets and charts.
• Report changes in residents' condition to the Nurse Supervisor promptly.
• Maintain confidentiality of resident information.
• Assist with daily living activities and mobility per care plan.
• Ensure rooms and beds are prepared for new/returning residents.
• Assist with meals, personal care, and daily routines.
• Follow policies on exposure to blood/body fluids.
• Observe for pressure areas and skin breakdowns and report.
• Participate in nursing reports and shift handovers.
