Job summary

Tufts Medicine Care at Home is seeking a part-time social worker to deliver clinical services across inpatient, ambulatory and community settings. You will assess patients and families, develop care plans, and coordinate with a multidisciplinary team to optimize outcomes.

Requires an MSW and LCSW with 1 year of related experience. Strong communication, documentation and advocacy skills are essential for successful collaboration with care providers and community resources.

Qualifications

• Master's in Social Work from an accredited institution.
• Licensed Clinical Social Worker (LCSW). Licensure in NH may be required depending on assigned territory and operational need.
• One year of related experience.

Responsibilities

• Provides psychosocial assessment of the individual and family, develops treatment plans with interventions.
• Provides psychosocial assessments in crisis situations.
• Assists with discharge planning to ensure continuity of care.
