Job summary

Tufts Medicine Care at Home is seeking a Hospice Social Worker (MSW/LCSW) in Lowell, MA, part-time 24 hrs/week. The role provides psychosocial assessments, care coordination, discharge planning, and family support across inpatient, ambulatory, and community settings within a multi-disciplinary team.

Responsibilities include crisis intervention, treatment planning, advocacy, and collaboration with health care providers and community resources.

Qualifications

• Master’s degree in Social Work from an accredited institution.
• Licensed Clinical Social Worker (LCSW).
• One year of related experience.

Responsibilities

• Perform psychosocial assessments of patients and families.
• Develop treatment plans and psychosocial interventions.
• Coordinate discharge planning and post-discharge support.
• Collaborate with care team and external resources.
• Advise and support patient advocacy and informed decision-making.
• Document thoroughly in medical records and compliance systems.
