Job summary

Tufts Medicine Care at Home, Inc. is seeking a Social Work professional to deliver comprehensive psychosocial assessments and discharge planning for patients and families in inpatient, ambulatory, and community settings.

The role requires an MSW with an LCSW license (NH may be required) and at least 1 year of related experience, with duties spanning crisis intervention, care coordination, and collaboration with multidisciplinary teams.

Qualifications

• Master’s degree in Social Work from an accredited institution.
• Licensed Clinical Social Worker (LCSW). Licensure in NH may be required depending on assigned territory and operational need.
• One (1) year of related experience.

Responsibilities

• Provides psychosocial assessment of the individual and family, developing treatment plans.
• Responsible for psychosocial assessments and interventions in crisis situations.
• Assists with discharge planning to ensure continuity of care and home/community support.
