Job summary

Liberty Hospice in Wilmington, NC seeks a Part-Time Palliative Care RN to coordinate patient and family care, review referrals, and provide pain and symptom management.

You will perform comprehensive assessments, support adherence to treatment plans, assist NPs, document in the EHR, and travel as needed to patient visits.

Applicants must have RN license in NC, at least 3 years of clinical nursing experience (home care/hospice preferred), CPR, and valid driver's license.

Qualifications

• Current RN licensure in the branch state and 3 years of clinical nursing experience.
• Experience preferred in home care, hospice, or skilled nursing facilities.
• Completed nursing education from an approved school of nursing.
• Ability to organize work independently, communicate patient needs, and coordinate visit schedules.
• Willingness to travel and work as assigned.
• Effective oral and written communication; CPR certification.
• Minimum 12 hours of continuing education per year; valid driver’s license.

Responsibilities

• Review referrals and coordinate care for patients and families.
• Conduct comprehensive physical and psychosocial assessments.
• Refer to specialists and support treatment plan adherence.
• Help patients access community services to improve health.
• Assist NPs with program tasks and palliative referrals tracking.
• Alleviate pain and distressing symptoms.
• Document all patient interactions in the EHR.
• Travel as required for patient visits.
