Qualifications

• Certification as an Adult or Family Nurse Practitioner.
• Experience in Hospice or Palliative Care preferred.
• Excellent communication and organization skills.

Responsibilities

• Implements palliative care program requirements.
• Completes medical visits with patients per guidelines and coordinates care with the Palliative Care team.
• Conduct face to face encounters to provide re-certification information to Medical Director/Associate Medical Director for Hospice patients.
• Provide clinical findings of that encounter to the primary care physician and supervising Medical Director/Associate Medical Director.
• Communicate with patients/families at all levels and with agency personnel.
