Job summary

Qualifications

• Current LPN licensure in NC and CPR certification.
• Ability to organize work and provide effective hospice care.

Responsibilities

• Provide direct patient care and coordinate services.
• Complete documentation per policy and regulation.
• Follow plan of care developed by the RN.
• Coordinate Hospice services with RN and care team.
• Accept and transcribe physician orders as directed.
• Forward clinical notes to RN for coordination of care.
• Attend and participate in IDT meetings.
