Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Fort Atkinson, WI. This PRN role supports ongoing resident care and collaborates with a multidisciplinary team to deliver high-quality therapy services.

Responsibilities include screening residents, implementing treatment plans, documenting progress, coordinating schedules, and ensuring CMS documentation standards are met.

Qualifications

• Associate's degree in Occupational Therapy.
• MUST have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention.
• Perform treatment plans; follow short- and long-term goals.
• Interpret and communicate evaluation findings to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned and as required to effectively treat patients.
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated.
