Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant to provide in-house therapy services at a Skilled Nursing Facility partner in Fort Atkinson, Wisconsin. This PRN position supports plan-oriented care under a supervising therapist.

Responsibilities include conducting screenings, implementing treatment plans, documenting progress, and collaborating with the interdisciplinary team to ensure quality resident care.

Qualifications

• Associate's degree in Occupational Therapy is required.
• MUST have active OTA licensure in the state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents to determine the best intervention or treatment for each patient.
• Implement treatment plans and work toward short- and long-term goals as directed by supervising therapist.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents and families/caregivers.
• Establish individual resident therapy schedules in cooperation with other team members.
• Provide therapy according to the treatment plan and ensure proper documentation and billing of services.
• Perform other duties as assigned to effectively treat patients and comply with CMS documentation standards.
