Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Fort Atkinson, WI. This PRN role supports resident care and collaborates with licensed therapists.

The OTA will plan, direct and deliver therapy services, conduct screenings, follow treatment plans, and document per CMS standards while pursuing ongoing professional development.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain current licensure and certification.

Responsibilities

• Screen residents to determine appropriate intervention plans.
• Implement treatment plans and follow short- and long-term goals.
• Interpret and communicate evaluation findings to residents and families.
• Schedule resident therapies in collaboration with team members.
• Deliver therapy per plan and document and bill CMS accordingly.
• Perform additional duties to effectively treat patients as required.
• Maintain CMS documentation standards daily with point-of-care records.
