Job summary

Evolve Therapy Services, LLC in Wisconsin is seeking a Certified Occupational Therapy Assistant to support an in-house Skilled Nursing Facility, on a PRN basis. You will plan, organize, and direct OT services to ensure high-quality resident care in compliance with standards and regulations.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain current licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention.
• Perform treatment plans and follow short- and long-term goals.
• Interpret and communicate evaluation findings to residents, families/caregivers, and the interdisciplinary team.
• Establish individual resident therapy schedules with other team members.
• Conduct therapy according to the treatment plan and ensure proper documentation and billing.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point-of-care documentation.
