Responsibilities

• Conduct regular resident screenings to determine best intervention.
• Implement treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings to residents, families and team.
• Schedule resident therapy in cooperation with team members.
• Deliver therapy per plan and ensure proper documentation and billing.
• Perform other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards daily with point-of-care documentation.
