Job summary

Responsibilities

• Follows the therapist's plan of care and communicates with the evaluating therapist before documenting in the daily note.
• Implements patient-centered, occupation-based intervention plans and adjusts treatment based on patient response and evidence.
• Demonstrates a sequential plan of treatment activities considering patient, family needs and time constraints.
• Adapts activities to meet physical, cognitive, and/or behavioral limitations and considers environment and outcomes.
• Collaborates with patient, family, and significant others throughout the occupational therapy process.
• Educates patients, families, caregivers, students, and other health care providers using effective teaching methods.
• Communicates progress with referring providers and other team members and documents clearly to support decisions for payers.
• Completes daily documentation and charging in therapy EMR and adheres to program requirements.
