Job summary

Roles involve hands-on personal care, mobility assistance, meal support, and documentation for quality care. Flexible schedules and opportunities for growth exist across shifts.

Qualifications

• Must-have: Age 18 or older.
• Active HHA or CNA certification or willingness to obtain certification.
• Ability to follow written care instructions and document services.
• Reliability, patience and professionalism with vulnerable populations.

Responsibilities

• Provide personal care including bathing, grooming, dressing, oral hygiene, and toileting.
• Assist mobility, transfers, ambulation, and ROM exercises per care plan.
• Prepare meals and assist with feeding as needed.
• Support medication self-administration through reminders.
• Monitor and report patient condition to supervising nurse.
• Take basic vital signs when trained.
• Maintain a clean, safe home environment for clients.
• Travel to patient homes and complete visits within service area.
• Complete required in-service training including dementia education.
