Job summary

Reid Health PACE in Indiana is seeking a compassionate Nursing Assistant to assist with nursing care, restorative therapies, activities, meals, and transportation across day center, home, and clinic. You will provide ADL and IADL support to frail elders and document care in the medical record.

Responsibilities include participating in interdisciplinary meetings, following CMS PACE guidelines and Reid Health PACE policies, and maintaining safe clinical areas and equipment for participants.

Qualifications

• Minimum 1 year of experience with frail or elderly populations.
• Preferred: 2 years in nursing homes or home health.

Responsibilities

• Assist with nursing care, restorative therapies, activities, meals, and transportation escort across care settings including day center, home, and clinic.
• Provides physical assistance and attendant care for activities of daily living and instrumental activities of daily living.
• Document in the medical record as appropriate to scope of practice.
• Participate in interdisciplinary meetings and communicate effectively.
• Maintain cleanliness and preparedness of exam rooms, day room, and kitchen and equipment.
• Follow Reid Health PACE policies, procedures, and OSHA safety guidelines.
