Job summary

Reid Health PACE Center is seeking a caregiving professional to assist with nursing care, restorative therapies, activities, meals, and transportation across care settings including day center, home, and clinic.

You will provide ADL/IADL support, participate in interdisciplinary meetings, and document care per scope of practice, while adhering to CMS PACE regulations and Reid Health policies.

Qualifications

• Minimum Education Required: NA
• Minimum Education Preferred: Current CNA certification in Indiana
• Minimum Experience Required: 1 year working with frail or elderly population
• Minimum Experience Preferred: 2 years in nursing home or home health environment

Responsibilities

• Assist with nursing care, restorative therapies, activities, meals, and transportation across care settings.
• Provide physical assistance and attendant care for ADLs and IADLs.
• Document in medical records within scope of practice.
• Communicate effectively in interdisciplinary meetings as required.
• Follow Reid Health PACE policies and OSHA safety guidelines.
