Reid Health’s PACE Center is seeking a Nursing Care Assistant - PRN to assist with daily nursing care across care settings, including day center, home, and clinic. Responsibilities include ADL support, meals, and transportation escort, with attention to safety and quality standards. Prior CNA certification in Indiana is preferred, with at least 1 year of elder care experience; CPR is a plus.

Schedule: Day Shift. PRN-as needed, hours 8a–4:30p.

Formación

• Current CNA certification in Indiana.
• Minimum of 1 year experience with frail or elderly population.
• CPR certification preferred but not required.

Responsabilidades

• Assist with nursing care, restorative therapies, activities, meals, and transportation escort across care settings.
• Provide ADL and IADL assistance and participate in interdisciplinary teams.
• Document in medical records and maintain safety guidelines per Reid Health PACE.
• Follow CMS PACE regulations and Reid Health policies; perform other duties as assigned.
