Nursing Care Assistant - PACE Home Health0106 PACE**Schedule:** Day Shift. PRN-as needed, Hours would be 8a-430p.**About the Position** Assist with nursing care, restorative therapies, activities, meals, and transportation escort assistance across care settings including but not limited to day center, home, and clinic. Provides physical assistance and attendant care for activities of daily living and instrumental activities of daily living to participants across care settings. Demonstrates the knowledge and skills necessary to participate in assessing, planning care for and providing all-inclusive service to frail elder enrollees according to assigned responsibilities and Reid Health PACE standards. Accepts other duties as assigned by the Clinical Manager or designee and within the NCA scope of practice **Overview of Responsibilities*** Assist with nursing care, restorative therapies, activities, meals, and transportation escort assistance across care settings including but not limited to day center, home, and clinic.* Provides physical assistance and attendant care for activities of daily living and instrumental activities of daily living to participants across care settings.* Accepts participant assignments by Day Center Supervisor and Home Care Coordinator.* Document in the medical record as appropriate to scope of practice.* Participate and effectively communicate in interdisciplinary meetings as required.* Act only within the scope of his or her authority to practice.* Follow all Reid Health PACE Policies and Procedures and OSHA safety guidelines.* Maintain safe working environment. Follows Safety Policies and Procedures.* Participate in continuing education classes and any required staff and training meetings. Maintain professional affiliations and any required certifications.* Participate in and support Quality Improvement initiatives.* Report abnormal findings or noted changes in participant status to the Clinical Manager or designee.* Assist in maintaining cleanliness and preparedness of exam rooms, day room, and kitchen and appropriate equipment.* Serve breakfast, snacks, and noon meal to all participants of Reid Health PACE according to diet orders.* Assist with the implementation of activities for participants tailored to the needs and limitations of frail elderly.* Handle potentially infectious specimens with appropriate biohazard precautions and practices standard precautions.* Perform other duties as required or requested within scope of practice.* Compliance with CMS PACE regulations as they pertain to this role.This list of duties and responsibilities is not intended to be all-inclusive and can be expanded to include other duties or responsibilities that management deems necessary **Education & Experience*** Minimum Education Required: NA* Minimum Education Preferred: Current CNA certification in Indiana* Minimum Experience Required: Minimum of 1 year working with a frail or elderly population* Minimum Experience Preferred: 2 years of experience working in a nursing home or home health environment.**Licensure & Certification**Licensure: None**Certifications:*** CNA certification preferred but not required can have on the job training for NCA with competency;* CPRReid Health's inpatient hospital is a 189-bed, regional referral center with a Level lll Trauma Center and a Primary Stroke Center designation.
