Job summary

Qualifications
- Graduate from an approved school of professional nursing and licensed to practice as an NP in the state.
- Active DEA license in the state where care is provided.
- CPR certification preferred.
- Must be a licensed driver who can travel to all business locations.
Responsibilities
- Develop and implement clinical plans of care for patients with chronic and terminal illnesses.
- Perform patient assessment, history and physical; manage symptoms.
- Monitor patient progress and communicate with the team; assist with discharge planning.
- Coordinate care with supervising physicians and other care teams.
- Prescribe medications as permitted by state regulations.
