Job summary

AccentCare in Killeen, TX is seeking a full-time Nurse Practitioner (Palliative Care) to join our clinical team. You will develop care plans for patients with chronic and terminal illnesses, perform assessments, and coordinate with physicians to provide compassionate, quality end-of-life care.

Qualifications include NP license in TX, CPR and DEA, at least 1 year of NP experience preferred, and willingness to travel to all locations served.

Qualifications

Licensed to practice as a Nurse Practitioner in the state of agency operation.
One year NP experience is preferred.
Active DEA license in the state where care is provided.
CPR certification preferred.
Must be a licensed driver who can travel to all business locations.
Meet state regulations for program services.

Responsibilities

Develop and implement clinical plans of care for patients with chronic and terminal illnesses.
Perform patient assessment, history and physical; recommendations on aggressive symptom-management.
Monitor patient progress; update Medical Director and team; assist in discharge planning and patient/family teaching.
Maintain communication with supervisory and attending physicians to review patient records and care plans.
Coordinate care with attending physicians and other staff to ensure optimal resource use for patients/families.
May provide support at time of death.
