Qualifications

• High school diploma or GED (or equivalent) is required.
• Three to six months of related care experience preferred.

Responsibilities

• Provides dementia care including adapted ADLs and meaningful engagement per resident’s service plan.
• Assists residents with daily living activities and helps with meals, mobility, and personal care as needed.
• Documents observations and communicates changes to supervisors.
• Maintains a clean, safe environment and follows safety procedures.
• Promotes socialization and participation in planned activities to support independence.
