Job summary

MedCentris Specialty Group seeks a licensed practical nurse to provide direct patient care under physician supervision in clinic and home settings.

You will administer medications, assist with wound care, document in the EMR, and participate in care planning and patient education. Travel to various clinic locations and home program visits may be required; nails policy and PPE guidelines apply.

Strong clinical skills and ability to work with multidisciplinary teams are essential.

Qualifications

• Graduate of accredited school of nursing.
• Current licensure with the State Board of Practical Nurse Examiners, with no restrictions.
• CPR/BLS required.
• TB and Flu vaccination.
• Physical requirements include mobility and lifting up to 25 pounds.

Responsibilities

• Observes, records and reports patient conditions to supervising MD/NP/PA.
• Administers prescribed medications and treatments per plan of care.
• Assists MD/NP/PA in specialized procedures and wound care.
• Prepares equipment, maintains sterile technique, and documents in EMR.
• Teaches and counsels patient and family on nursing care needs.
• Completes daily paperwork and photo documentation of wounds.
• May travel to clinic locations and home visits for care.
• Performs routine venipuncture if competency documented.
