Job summary

Reliant Care Management Company is seeking a licensed practical nurse to provide compassionate patient care and support to residents and staff. You will conduct assessments, follow care protocols, document changes, and collaborate with the medical team to ensure quality outcomes.

Responsibilities include admitting assessments, discharge planning, wound care, and leading CNAs/CMTs to uphold facility standards.

Qualifications

• Must hold LPN licensure and be in good standing.
• Experience with geriatric or long-term care preferred.
• Strong assessment and documentation skills.

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation
• Provide wound care: follow protocols, irrigate, pack, and remove dressings
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies
• Respond effectively to emergency situations
