Polaris Rehabilitation & Care Center in Cheyenne, WY is seeking a compassionate Licensed Practical Nurse (LPN) to join our clinical team. You will provide direct nursing care, administer medications, monitor residents, and support safe, high-quality, resident-centered care.

You will collaborate with RNs, nursing assistants, physicians, and inter-disciplinary team members to promote positive clinical outcomes and meaningful resident relationships in a supportive environment.

Qualifications

• Graduate of an approved practical nursing program.
• Current, active LPN license in good standing in Wyoming.
• Current CPR or BLS certification.
• Experience in skilled nursing, rehab, long-term care preferred.

Responsibilities

• Deliver direct nursing care per care plans and physician orders.
• Monitor resident conditions and report changes to the RN or clinical team.
• Administer medications and treatments within scope of practice.
• Assist with care coordination, assessments, and daily operations.
• Supervise and support Certified Nursing Assistants and direct staff.
• Communicate respectfully with residents, families, and team members.
• Document accurately in the electronic medical record.
