Job summary

Atrium Health Navicent's Carlyle Place in Macon, GA is seeking an LPN to provide nursing care across Skilled Nursing, Assisted Living and Memory Care settings. You will work three 12-hour day shifts (7:00 AM–7:00 PM) with every other weekend, participate in planning resident care, administer medications, and collaborate with the interdisciplinary team to ensure comfort and quality outcomes.

Qualifications

• Graduate of an accredited School of Practical Nursing.
• Current, unrestricted Georgia LPN license (or compact license eligible).
• Current CPR certification or ability to obtain within 60 days of hire.
• Basic Life Support (BLS) certification required per policy.
• Skilled nursing, long-term care, assisted living, or memory care experience preferred.

Responsibilities

• Provide compassionate nursing care to residents in Skilled Nursing, Assisted Living, and Memory Care settings.
• Monitor resident conditions and report significant changes to the healthcare team.
• Assist with assessments, care planning, treatments, and procedures.
• Administer medications and treatments within scope of practice.
• Educate residents and families regarding care plans and treatments.
• Document assessments, interventions, and resident responses accurately and timely.
• Collaborate with interdisciplinary team members to support positive resident outcomes.
• Ensure care is delivered safely, efficiently, and in accordance with regulatory requirements.
