Atrium Health Navicent is seeking an LPN to join Carlyle Place in Macon, GA, for night shifts. You will provide nursing care across Skilled Nursing, Assisted Living, and Memory Care, under RN supervision, with a focus on comfort and quality outcomes.

You will administer medications, assist with assessments, and educate residents and families while documenting care and coordinating with an interdisciplinary team. Strong licensure and BLS/CPR are required or within 60 days of hire.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current, unrestricted Georgia LPN license (or compact license eligible).
• CPR and BLS certifications required or within 60 days.
• Experience in skilled nursing, long-term care, or memory care preferred.

Responsibilities

• Provide nursing care across Skilled Nursing, Assisted Living, and Memory Care.
• Monitor resident conditions and report changes.
• Assist with assessments, care planning, treatments, and procedures.
• Administer medications within scope of practice.
• Educate residents and families about care plans.
• Document assessments and resident responses.
• Collaborate with interdisciplinary team to support outcomes.
• Ensure safe, compliant care with regulatory requirements.
