Atrium Health Navicent's Carlyle Place in Macon, GA seeks a Licensed Practical Nurse (LPN) for night shift across multiple care settings. You will provide hands-on nursing care, participate in care plans, and coordinate with a diverse healthcare team to ensure resident comfort and safety.

The role requires an accredited Practical Nursing program graduate with GA LPN licensure, CPR and BLS certifications, and preferred experience in skilled nursing or memory care.

Qualifications

• Graduate of an accredited School of Practical Nursing.
• Current, unrestricted Georgia LPN license (or compact license eligible for Georgia).
• Current CPR certification or ability to obtain within 60 days of hire.
• Basic Life Support (BLS) certification required per policy.
• Skilled nursing, long-term care, assisted living, or memory care experience preferred.

Responsibilities

• Provide compassionate nursing care to residents in Skilled Nursing, Assisted Living, and Memory Care settings.
• Monitor resident conditions and report significant changes to the healthcare team.
• Assist with assessments, care planning, treatments, and procedures.
• Administer medications and treatments within scope of practice.
• Educate residents and families regarding care plans and treatments.
• Document assessments, interventions, and resident responses accurately and timely.
• Collaborate with interdisciplinary team members to support positive resident outcomes.
• Ensure care is delivered safely, efficiently, and in accordance with regulatory requirements.
