Atrium Health Navicent’s Carlyle Place in Macon, GA seeks a Licensed Practical Nurse (LPN) to care for residents across Skilled Nursing, Assisted Living and Memory Care. You’ll join a collaborative team devoted to comfort, wellness and quality outcomes under RN supervision.

The role covers assessments, treatments, medication administration, care planning, and education for residents and families. A Georgia LPN license, CPR/BLS and nursing experience in long-term or memory care are preferred.

Qualifications

• Graduate of an accredited School of Practical Nursing.
• Current, unrestricted Georgia LPN license.
• Current CPR certification or ability to obtain within 60 days of hire.
• Basic Life Support (BLS) certification required per policy.
• Skilled nursing, long-term care, assisted living, or memory care experience preferred.

Responsibilities

• Provide compassionate nursing care to residents in Skilled Nursing, Assisted Living, and Memory Care settings.
• Monitor resident conditions and report significant changes to the healthcare team.
• Assist with assessments, care planning, treatments, and procedures.
• Administer medications and treatments within scope of practice.
• Educate residents and families regarding care plans and treatments.
• Document assessments, interventions, and resident responses accurately and timely.
• Collaborate with interdisciplinary team members to support positive resident outcomes.
