Job summary

Qualifications

• Current LPN license to practice in the state where the community is located.
• Experience with seniors, preferably in assisted living or LTC.
• Able to work in a team and supervise staff.
• Strong time management and organizational skills.
• Good physical condition; able to lift up to 50 lbs.
• Willing to work an 8- to 16-hour flexible schedule including weekends and holidays.
• Certified or willing to take required state certification.

Responsibilities

• Provide quality nursing care and services to residents.
• Coordinate physical, social, and emotional aspects of care per physician orders and resident choices.
• Execute the medication management plan and communicate changes to the physician.
• Assess resident needs and update appropriate individuals on changes in condition.
• Ensure resident wellness and safety with proper documentation.
• Collaborate with team to meet resident needs and respond to emergencies promptly.
• Report incidents and changes to Healthcare Director with appropriate documentation.
• Assist with meal delivery and monitor diet acceptance and nutrition.
• Maintain accurate records and participate in shift change reporting.
• Motivate and direct CNAs/RMAs and attend required trainings.
