Job summary

Midwest Health, Inc. is seeking a Licensed Practical Nurse to provide direct nursing care to residents in our community. You will follow federal, state, and local standards and work under the RCC Services or Nurse Supervisor to maintain high-quality care.

Core duties include assisting in admissions and discharges, daily hygiene and mobility support, documentation, and ensuring confidentiality. LPN licensure and English proficiency are required, and you may be asked to work beyond regular hours

Qualifications

• Must hold a valid LPN license in good standing with the state.
• Basic computer skills; ability to read, write, speak and understand English.
• Ability to work extended hours, including weekends and holidays, when required.

Responsibilities

• Provide direct nursing care to residents in accordance with federal, state, and local standards and guidelines.
• Comply with the Community’s policies and procedures and nursing department rules.
• Report abuse/neglect immediately to the appropriate supervisor and maintain resident dignity and privacy.
• Maintain confidentiality of resident and family information and document changes in a timely manner.
• Assist in admission and discharge of residents and support daily hygiene, bathing, and personal care.
• Assist with transferring, lifting, and positioning residents; transport to appointments and activities.
• Document care plans, charts, and reports; complete accident/incident reports as needed.
• Oversee care quality, communicate with team members, and ensure safe operation of equipment.
