Bethesda Health Group seeks a dedicated RN to accurately assess residents' health status on admission and ongoing care, communicate changes to the interdisciplinary team, and document assessments and care plans. You will implement nursing interventions, administer medications and IVs, and use SBAR to notify physicians while maintaining safety and quality in a dynamic care environment.

Required is an accredited nursing program graduate with an active RN license in Illinois and CPR/AED.

Formación

• Long-term care experience preferred.

Responsabilidades

• Assess residents' health status on admission and ongoing basis.
• Remains aware of and communicates significant changes to the IDT.
• Accurately documents assessments and care plans.
• Develops dynamic care plans with physicians, residents, families, and other IDT members.
• Reviews and revises the care plan as needs change.
• Documents interventions, including medications, treatments, and IVs.
• Uses SBARR for timely physician notification.
• Maintains composure during crises and supports resident safety.
• Assists with ADLs and responds to call lights as needed.

Accredited nursing school graduate resulting in RN
