Job summary

Monarch Healthcare Management is seeking an LPN to provide residents with high-quality direct routine care under nursing leadership. The LPN participates in developing and modifying the tenant Plan of Care and implements interventions delegated by the RN or physician.

The LPN collects resident health data, reports changes to the RN, and assigns and monitors nursing tasks for unlicensed staff to ensure residents’ well-being.

Qualifications

• Completion of a professional nursing education program approved by the Minnesota Board of Nursing
• Current Minnesota Licensure as a Practical Nurse
• Current CPR certification and emergency care

Responsibilities

• The LPN provides residents with direct routine care under nursing leadership.
• Participates in developing and modifying the Plan of Care and implements delegated interventions.
• Collects resident health data and reports changes to the RN.
• Assigns and monitors nursing tasks or activities of unlicensed staff, clarifying expectations and escalating as needed.
• All nursing activities support residents’ physical, mental and psychosocial well-being.
