Job summary

Monarch Healthcare Management seeks a Licensed Practical Nurse (LPN) to provide direct routine care to residents under nursing leadership at our Minnesota facilities.

You will participate in care planning, monitor health data, and implement delegated interventions. The LPN will supervise unlicensed staff and collaborate with RNs to support residents' physical, mental, and psychosocial well-being.

Qualifications

• Minnesota license as a Practical Nurse (LPN) required.
• Current CPR certification required.

Responsibilities

• Provide residents with direct routine care under nursing leadership.
• Assist in development and modification of the Plan of Care and implement delegated interventions.
• Collect health data, compare to normal findings, and report changes to the RN or physician.
• Monitor and supervise nursing tasks performed by unlicensed staff; clarify expectations as needed.
• Support residents' physical, mental, and psychosocial well-being to the highest practicable level.
