Job summary

Rivaya of Des Plaines is seeking dynamic and compassionate LPNs to join our Extended Care affiliated facility. You will provide focused, quality-driven nursing care to residents, direct nursing assistants, and participate in a multidisciplinary team to ensure safety and well-being.

This full-time role requires a valid state nursing license, ability to work every other weekend, and strong communication. We offer a comprehensive benefits package including health, dental, vision, 401(k), PTO, paid

Qualifications

• Valid Nursing License as required by the state.
• Ability to work every other weekend.
• Compliance with the state’s Health Care Worker Background Check requirements.
• Relate positively and favorably to residents and visitors.
• Ability to undertake diverse care tasks within a multidisciplinary team.
• Familiarity with hospital health, safety, and sanitation standards.
• Understanding of confidentiality and nursing best practices.
• Excellent communication and interpersonal skills.
• Energetic, professional, and reliable.

Responsibilities

• Direct the day-to-day activities of the nursing assistants.
• Complete required record-keeping upon resident admission, discharge, and transfer; chart and document per facility policy.
• Prepare and administer medications as ordered by a physician and perform other drug administration functions per established policies.
• Make daily rounds of the unit and prioritize tasks in a fast-paced environment.
