Riverton Rehabilitation and Healthcare Center is seeking a dedicated LPN to provide compassionate, high-quality nursing care in a sub-acute and long-term setting. The role supports residents’ physical and emotional needs under the guidance of a nurse manager and interdisciplinary team.

Qualifications

• Current, unencumbered PA LPN license.
• BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Provide patient care in accordance with standards and plan of care.
• Follow and implement individualized care plans by Nurse Manager and team.
• Perform medication administration and treatments per plan and policies.
• Monitor residents’ conditions and report changes promptly to supervising staff.
• Maintain accurate documentation per orders and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and families.
